CIC's core platform, Care Cohort, is achieving this by providing the resident, family and care staff health and wellness information, and actively engaging the care cohort into the care outcomes. Via any smart device, the care cohort can be alerted to changes in a person's health trends, and can then take proactive steps to improve the care outcomes.
Living Longer and Living Better is achieved through video and telehealth, reduction in social isolation through community on line games, a common calendar, medication reminders, health information for daily activities, entertainment such as Movies on Demand, health and wellness information from wearable and passive sensors.
